Formula for Forms

Hey, Y'all,
Well, this morning I have been filling out medical forms in preparation for my eye appointment day after tomorrow. Do you dislike that little chore (I say little with tongue in cheek) as much as I do? I have been looking at the folder with the papers in it for the last week. They very kindly mail it about ten days in advance, so that patients can fill out the forms and bring them when they come for the first visit. 

You know, they send two papers that are almost identical in what they ask. Medical history: medications, illnesses, surgeries are all in duplicate. Two different papers- same information asked a little differently. I decided today that I would put all that information into a microsoft form on my computer, and attach it to the forms they have sent me. Yes, I filled it in on the forms they sent me, except for the surgeries, and just wrote (see attached forms) and stapled them to their forms. You see, I have had a total of sixteen surgeries over the past fifty-nine years. Plus a few more minor ones. 

Well, it is finished for now and saved on my computer. My advice to any of you who take several medications and have had several surgeries to do the same and then when you have to go to a new doctor, you can just update it and take it with you. It saves a lot of time and extra thought. If you have ever gone to the doctor and have to sit there and try to remember stuff, these lists that you have prepared in advance are quite helpful. It is much easier to think of it at home and take it with you. 

Would you believe that it was 9 a.m. before I got up this morning? Yep, I went to bed around 10 pm and left the television off. Slept until 3:30, woke up, and went back to sleep until nearly 5:00. I turned on the tevee, and was back asleep by around 5:45 and slept until 9. So, I really got a goodly amount of sleep. It would be wonderful to sleep 8 hours in a row, but I know that is too much to ask. I am just grateful for the sleep I did get. 

I have some more quilt work to do this afternoon. Tomorrow I am going to the church to begin work on the church music inventory for Karen, the Choir Director. My kitchen is basically clean, my floors are in good shape, and my paperwork is done and filed. Whoopee! I got to spend some time online with my daughter, Teresa, and all is well in my bailiwick. I got to watch the next to last episode of "The Closer" last night and am in suspense over how it will end next week.

I am not a lover of form filling.And what heaven it would be to get a full 8 hours sleep.
